Description

Compazine is an antipsychotic and antiemetic medication. It contains the active ingredient Prochlorperazine which belongs to a drug group called phenothiazines. Prochlorperazine works by blocking the action of a chemical called dopamine which acts on the nervous system in the brain.

Compazine is usually prescribed for the treatment of severe nausea and vomiting caused by different conditions including radiation therapy, cancer chemotherapy, surgery, etc. It has the ability to stop the feeling of dizziness and vertigo associated with Meniere’s disease, Labyrinthitis, and migraine.

Owing to the antipsychotic effect of Prochlorperazine, the medication is used for the management of the manifestations of psychotic disorders such as schizophrenia. Compazine can also be used as a short-term treatment of generalized non-psychotic anxiety.

Indications

Control of severe nausea and vomiting caused by radiation therapy, cancer chemotherapy, surgery, etc.

Relief during nausea, vomiting, dizziness, and vertigo associated with Meniere’s disease, Labyrinthitis, and other inner ear disorders.

Management of the manifestations of psychotic disorders such as schizophrenia.

Short-term treatment of generalized non-psychotic anxiety.

MOA (mechanism of action)

Prochlorperazine is a phenothiazine antiemetic of the first generation antipsychotic group. It blocks the D2 somatodendritic autoreceptor which results in the closure of postsynaptic dopamine receptors in the mesolimbic system and an increased dopamine turnover providing anti-dopamine action. The dopamine blockade in the chemoreceptor trigger zone leads to anti-emetic effects of Prochlorperazine. It also blocks anticholinergic and alpha-adrenergic receptors which leads to sedation, muscle relaxation, and hypotension.

Dosage

Follow all the instructions given by your doctor. Do not use Compazine in larger or smaller amounts or for longer than recommended. If you take this medication long-term, you may need frequent medical tests.

The medication is taken orally. Place the tablet high up along your top gum under the upper lip. Allow the tablet to dissolve completely which will take 1-2 hours. Do not move the tablet about the mouth. Do not crush, chew or swallow the tablet. It is recommended to take Compazine after meals.

Maximum dose must not exceed 40 mg per day.

The medication should be used with caution in elderly patients with psychotic disorders. It is recommended to start the treatment with lower doses.

Missed dose

If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose to make up for a forgotten one.

Overdose

A large dose of Compazine may lead to central nervous depression progressing from drowsiness to coma with areflexia. Patients with mild intoxication may experience restlessness, confusion, and excitement.

If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately. Do not induce emesis.

Storage

Store at room temperature between 20°C and 25°C (68°F and 77°F) away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Side effects

The most common side effects associated with Compazine are:

  • tardive dyskinesia
  • neuroleptic malignant syndrome (NMS)
  • hypotension
  • agranulocytosis
  • neutropenia
  • angina
  • tachycardia
  • constipation
  • dry mouth
  • drowsiness
  • akathisia
  • parkinsonism
  • blurred vision

Side effect occurrence does not only depend on medication you are taking but also on your overall health and other factors.

Contraindications

Do not take Compazine if you’re allergic to Prochlorperazine or any other ingredients present in the formulation.

Do not take Compazine if you have Parkinson’s disease, Reye’s syndrome, pre-existing central nervous system depression or coma, bone marrow suppression, or pheochromocytoma.

Do not take Compazine during intrathecal radiographic contrast administration.

Do not consume alcohol during the first weeks of Compazine treatment.

Compazine is not recommended for use in patients under the age of 2 and under the weight of 9 kg.

Compazine is not recommended for use in pregnant or breastfeeding women.

Sudden termination of Compazine treatment may cause adverse effects. Stop the treatment gradually, by lowering the dosage.

Do not drive after taking Compazine unless you already know how you react to the use of this medicine.

Consult a doctor before starting Compazine treatment if you have:

  • Serious cardiac or pulmonary disease.
  • Siabetes mellitus.
  • Thyroid disease; malnutrition.
  • Alcoholism.
  • Hepatic disease.
  • Seizure disorder.
  • Glaucoma, prostatic hypertrophy, urinary retention.
  • Gastrointestinal obstruction, ileus.
  • Risk factors for a blood clot such as high blood pressure or high cholesterol levels.
  • History of blood clots.
Drug interactions

Do not co-administrate Compazine with any of the following medicinal products:

  • class IA antiarrhythmic agents (quinidine and disopyramide)
  • class III antiarrhythmic agents (amiodarone and sotalol)
  • other medications such as bepridil, cisapride, sultopride, thioridazine, methadone, intravenous erythromycin and vincamine, halofantrine, pentamidine, sparfloxacin
  • medicines which induce bradycardia like bradycardia-inducing calcium channel blockers (diltiazem, verapamil), beta-blockers, clonidine, guanfacine, digitalis
  • medicines which can cause hypokalaemia such as diuretics, stimulant laxatives, intravenous amphotericin B, glucocorticoids, tetracosactide
  • other antipsychotics
